don't know how they are doing medical clearances now.....I'd email your son and specifically ask ASAP

we did not unless there was a medical condition that could have impacted the cruise...I've heard that has changed and you MAY need to get signed off by a doctor....best to ask your sailor for an exact clarification

do these forms have to be signed by a doctor? we are planning to take TC on the reagan.

Just got my form from my son for the upcoming cruise on the USS Nevada SSBN-733. It is even simpler then yours, only 1 1/2 pages, 9 questions. Basic stuff like "Can you climb a 20 ft vertical ladder?" Various disease questions and what med are you on. And no, I am not pregnant!
